Admissions Forms

Applicants and students of Georgia Piedmont Tech will need the following forms to process various requests. It is strongly recommended that you keep a copy of your requests for your personal records, especially those submitted via email. When emailing requests, your Georgia Piedmont Tech email should be used to prevent a false spam flagging. All requests submitted to the Admissions office will be processed within 3-5 business days unless otherwise noted. If you have any further questions, please contact the Office of Admissions.

Applicants who need to prove Georgia residency for instate tuition consideration must complete the Applicant Petition for Georgia Residence Classification form and supporting documents. Petitions must be approved before the first official day of the semester to be effective that semester.

Application Verification RequestFuture StudentsAdmission Documents

Applicants who need to verify that an application for admission has been submitted to Georgia Piedmont Tech must complete and email the Application Verification Request form to the Office of Admissions. Once submitted, the Office of Admissions can either mail the document to the requested institution or hold it to be picked up by the applicant.

GED Transcript RequestFuture StudentsAdmission Documents

Students, who passed the GED Test in the State of Georgia, may request their test scores using the GED Official Transcript/Diploma Request form.

  • There is a $15.00 fee for a GED transcript and a $15.00 fee for a GED Diploma. The form should be completed online and then printed to forward to the Technical College System of Georgia (TCSG).
  • The form can be delivered or mailed to TCSG with a cashier’s check or money order to the address listed on the top of form. Personal checks and faxed requests are not accepted. After the request is received by the Georgia GED Testing Program, please allow 7 business days for processing.

Applicants who took the GED Test outside the State of Georgia can request their transcript from the American Council on Education (ACE). Please be aware that there is a fee required to obtain a copy of an official transcript through ACE.

High School/Post-Secondary Transcript RequestFuture StudentsAdmission Documents

Applicants to Georgia Piedmont Tech must submit official transcripts from the last high school attended showing the date of graduation, or submit official GED transcripts and official transcripts from each college or university attended.

Complete the online High School/Post-Secondary Transcript Request form and print to forward to the high school/post-secondary institution. It is important to include former names to expedite the processing of your documents and the completion of your admissions file. The High School/Post-Secondary Institution may charge a processing fee for each official transcript requested by a student. Contact the institution directly for payment and submission guidelines.

Name Change RequestFuture StudentsAdmission Documents

Students who are submitting documentation verifying a legal name change should follow the instructions below after completing the Name Change Request form:

  • Email the completed form to the Office of Admissions at admissions@gptc.edu, using their Georgia Piedmont Tech student email
  • Hand deliver or mail both the following original documents to the Admissions Office (original documents only; copies will not be accepted. The Office of Admissions will make copies and return the originals to you.):
    • Official Court order or Marriage License
    • Social Security Card with new name
Program Change RequestFuture StudentsAdmission Documents

Before submitting the Program Change Request form students must be sure they:

  • submit the form by the semester application deadline.
  • meet the admissions requirements for the requested program. Accucplacer and Asset test scores and high school diploma graduation status will be evaluated by the Admissions Office prior to approving the change.
  • visit the Student Financial Services Office to discuss the impact of the program change on your financial aid before you submit this request to the Office of Admissions. Program changes may be limited and/or may affect your financial aid.

Processing Fee:
$10.00 per Program Change Request

Pay by Fax:
After the form is completed, print and fax it to the Cashier Office at (404) 298-5235. Make sure all questions in the Debit/Credit Card Authorization section are filled out completely.

Pay in Person

After the form is completed, print and hand deliver to the Cashier Office with either cash, check, money order or credit/debit card.

Request for Placement Test ScoresFuture StudentsAdmission Documents

To request an official copy of your placement test scores that you received from an assessment instrument administered by Georgia Piedmont Tech, complete the Request for Placement Test Scores form. Please allow 7-10 business days for processing, and an additional 7 business days for receipt if scores are to be received by an alternative institution.

Processing Fee:
$5.00 per Request for Placement Test Scores

Pay by Fax:
After the form is completed, print and fax it to the Cashier Office at (404) 298-5235.
Make sure all questions in the Debit/Credit Card Authorization section are filled out completely.

Social Security Change RequestFuture StudentsAdmission Documents

Students requesting a change in the social security number in their admissions file must follow the instructions below after completing the Social Security Change Request form:

Send the completed form to the Office of Admissions at admissions@gptc.edu

Mail:
Hand deliver or mail the Social Security Card to the Admissions Office. Copies of the Social Security Card will not be accepted. The Office of Admissions will make copies and return the originals to you.

Student Information Change RequestFuture StudentsAdmission Documents

Students should submit a completed Student Information Change Request form to the Office of Admissions at admissions@gptc.edu (using their Georgia Piedmont Tech student email) to update the following information:

Semester Update
You applied but did not attend the semester you applied for and would like to attend an upcoming semester within the next year.

File Status Change
You are currently a provisional student and have taken a Accuplacer test or completed the required credits or courses to qualify you to change your status to regular.

Campus Change

You desire to change your Georgia Piedmont Tech campus.

Address Change
You would like to change your address; all mail sent to you by Georgia Piedmont Tech will be mailed to the new address.
