Admissions Forms

The Office of Admissions at Georgia Piedmont Technical College provides applicants and students access to on-line forms that are required for various processing requests. Before clicking on the link to complete a request form, please read the instructions carefully to ensure accurate and timely processing.

NOTE: You may use the computers in the Admissions/Registrar's Lobby or the Computer Lab to complete a request form. Once you complete the form, you will need to save the XML file to your USB and then send it as an attachment from your Georgia Piedmont Tech Student Email (preferred) or personal email to admissforms@gptc.edu. For your privacy, DO NOT SAVE INFORMATION ON THE HARD DRIVE OF ANY COMPUTER DESIGNATED FOR STUDENT OR PUBLIC USAGE. If you have difficulty accessing your Student Email, please contact Student Support at 404-297-9522 ext. 5399.

It is strongly recommended that you print a copy of your request, especially for those being submitted to the Admissions Office via email. To guarantee our receipt of request forms that are submitted via email, the Georgia Piedmont Tech Student Email should be used. Other email accounts have the possibility of being identified as spam. Also note that all forms require Adobe Acrobat Reader.

All requests submitted to the Admissions Office will be processed within 3-5 business days unless otherwise noted. If you have questions or experience difficulty completing any of the on-line forms, please contact the Office of Admissions at 404-297-9522, ext. 1602 or admissions@gptc.edu.

Program Change Request
Effective October 2, 2010, the Admissions Office will charge a processing fee of $10.00 for each Program Change Request.
Students will have the option of paying via fax or in person.

  • Pay by Fax
    If paying by fax, students may pay by credit/debit card only. After the form is completed, print and fax it to the Cashier Office at 404-298-5235. Make sure that all questions in the Debit/Credit Card Authorization section are filled out completely.

  • Pay in Person
    Students submitting a Program Change Request in person will have the option of paying with cash, check, money order or credit/debit card. After the form is completed, print and hand deliver to the Cashier Office with your form of payment.

It is the responsibility of the student to consult with the Student Financial Services Office to determine if a change in financial aid will occur because of the Program Change Request. Once you have consulted with the Student Financial Services Office, complete the Program Change Request form. Your request will be processed within 3-5 business days.

Name Change Request
Students who are required to submit documentation verifying a legal name should follow the instructions below.

  1. Complete and submit the Name Change Request form to the Admissions Office via email (click Submit by Email on the form).
  2. Hand deliver or mail the following original documents to the Admissions Office:
    1. Official Court order or Marriage License (we will copy them) and
    2. Social Security Card with new name

Copies of these documents will not be accepted. The Admissions Office will make copies from the original documents and return the original documents to the students.

Student Information Change Request
The Student Information Change Request form is used for updating your semester, changing your file status, making a campus change and changing your address. Students are required to submit the form via email from your Georgia Piedmont Tech Student Email using the Submit by Emailbutton located on the form. Former students who have not taken classes for more than one year must fill out a new application by the deadline if you plan to take classes for the upcoming semester. Please be sure all information is correct before clicking the Submit by Email button.

The information below provides helpful information regarding student status and the changes that can be requested using the Student Information Change Request form.

Student Status:

  1. Applicant – You have a current application on file but have not started classes.
  2. Current – You are currently taking classes, or have taken classes within the past year.
  3. Former – You have not taken classes within the past year.

Change Requested:

  1. Semester Update – You applied but did not attend the semester you applied for and would like to attend an upcoming semester within the next year.

  2. File Status Change – You are currently a provisional student and have taken a Compass test or completed the required credits or courses to qualify you to change your status to regular.

  3. Campus Change – You desire to change your Georgia Piedmont Technical College campus; you may select the Clarkston or the Covington campus.

  4. Address Change – You would like to change your address; all mail sent to you by Georgia Piedmont Tech will be mailed to the new address.

Application Verification Request
Applicants may complete an Application Verification Request form if a document is needed to verify that an application for admission has been submitted to Georgia Piedmont Technical College. The Admissions Office will process Admissions Verification Requests that are received via email by clicking the Submit by Email button on the on-line Application Verification Request form. Once the request has been completed, the Admissions Office can either mail the document to the receiving institution or prepare the document for pick up by the applicant. There is no fee required for this request. Applicants should complete a separate form for each institution receiving the Application Verification.

Request for Placement Test Scores
To request an official copy of your placement test scores that you received from an assessment instrument administered by Georgia Piedmont Technical College, complete the Request for Placement Test Scores form. The Admissions Office charges a processing fee of $5.00 for each official Placement Test Score Request.

Students will have the option of paying via fax or in person.

  • Pay by Fax

  • Pay in Person
    Students submitting a Request for Placement Test Scores in person will have the option of paying with cash, check, money order or credit/debit card. After the form is completed, print and hand deliver to the Cashier Office with your form of payment.

NOTE: Please allow 7-10 business days for processing, and an additional 7 business days for receipt, if scores are to be received by an alternative institution.

GED Transcript Request
Students, who passed the GED Test in the State of Georgia, may request their test scores using the GED Official Transcript/Diploma Request form. There is a $15.00 fee for a GED transcript and a $15.00 fee for a GED Diploma. The form should be completed on-line and then printed to forward to the Technical College System of Georgia (TCSG).

The form can be delivered or mailed to TCSG with a cashier's check or money order to the address listed on the top of form. Personal checks and faxed requests are not accepted. After the request is received by the Georgia GED Testing Program, please allow 7 business days for processing.

Applicants who took the GED Test outside the State of Georgia can request their transcript from the American Council on Education (ACE) by visiting www.acenet.edu. Please be aware that there is a fee required to obtain a copy of an official transcript through ACE. Please post this information on 2/1/14.

High School/Post Secondary Transcript Request
Applicants for admission to a credit program at Georgia Piedmont Technical College must submit official transcripts from the last high school attended showing the date of graduation or submit official GED transcripts and official transcripts from each college or university attended. The High School/Post Secondary Transcript Request form is provided to assist applicants with requesting copies of transcripts from other institutions.

To request an official transcript from another institution:
Complete the on-line High School/Post Secondary Transcript Request form and print to forward to the high school/post secondary institution. It is important to include former names to expedite the processing of your documents and the completion of your admissions file.
The High School/Post Secondary Institution may charge a processing fee for each official transcript requested by a student. Contact the institution directly for payment and submission guidelines.

Additional Information
For GED test scores, applicants should complete a GED Official Transcript/Diploma Request form. Current students may click here to obtain information about requesting an official Georgia Piedmont Technical College Transcript.

Social Security Change Request
Students requesting a change in the social security number in their admissions file must follow the instructions below.

  1. Complete and submit the Social Security Change Request form to the Admissions Office via email (click Submit by Email on the form).

  2. Hand deliver or mail the Social Security Card (with new name) to the Admissions Office.

  3. On-line Students may mail the Social Security Card to the appropriate campus:
